All-on-4® in Charlotte, NC

When you are missing most or all of your teeth, every meal, conversation, and photo becomes a reminder of what you no longer have. Removable dentures offer a visible solution, but they shift against the gums with every bite, rely on adhesives to stay in place, and allow the underlying jawbone to continue shrinking, which means the fit gets worse over time rather than better. All-on-4 dental implants in Charlotte replace both the teeth and the root structure beneath them, giving you a fixed, bone-supported result that functions the way a natural smile does.

Harmony Dental Implants and Oral Surgery is one of the few Charlotte-area practices where a board-certified oral and maxillofacial surgeon performs every stage of your All-on-4 care in-house, from the CBCT imaging and treatment planning through the surgical placement and final restoration. Dr. Riddhi Patel earned her DMD from the University of Pennsylvania, completed her surgical residency at Mount Sinai Hospital in New York, and is board-certified by the American Board of Oral and Maxillofacial Surgery. She is a member of the American Association of Oral and Maxillofacial Surgeons, the American College of Oral and Maxillofacial Surgeons, and both the North Carolina and South Carolina Dental Societies. For all patients pursuing dental implants at our Charlotte practice, we warranty every implant for five years and will replace it at no cost if a failure occurs.

What Are All-on-4 Dental Implants?

All-on-4 is a full-arch replacement system that uses four strategically placed titanium implants to support a complete set of upper or lower teeth. Two implants are positioned vertically at the front of the arch, and two are angled at the rear to maximize available bone contact, which allows the system to work even when some bone loss has already occurred. A temporary fixed arch is attached the same day as surgery, so most patients leave our Charlotte office with functional teeth before their permanent restoration is placed months later.

Long-term clinical data backs how dependable this approach is. A study published through the National Institutes of Health documented All-on-4 cumulative implant survival rates of 96.2% over ten years, with the maxillary arch showing 95.7% survival at 13 years. For patients looking for a permanent solution rather than a temporary fix, All-on-4 is one of the most extensively studied full-arch options available.

All-on-4 vs. Traditional Dentures

The core difference between All-on-4 and conventional dentures is what happens beneath the gum line. Dentures sit on top of the gums and do nothing to stop the bone from resorbing, which is why the fit deteriorates year after year and requires repeated relines. All-on-4 anchors into the bone itself, which stimulates it and keeps the restoration stable without adhesives.

Are You a Candidate for All-on-4 in Charlotte?

Most healthy adults who are missing all or most of their teeth on one or both arches qualify for All-on-4. The angled placement of the posterior implants allows the system to work with less bone than conventional implant protocols require, which opens the door for patients who have been turned away elsewhere due to bone loss. Dr. Patel evaluates every case with CBCT imaging before making any recommendation. Common candidacy factors include the following:

Missing Most or All Teeth

Adults missing all or most teeth on one or both arches due to decay, gum disease, or long-term tooth loss are the primary candidates for full-arch restoration.

Bone Loss Present

All-on-4’s angled posterior placement maximizes contact with available bone, making it viable even after significant resorption from denture wear or prolonged tooth loss.

Transitioning From Dentures

Current denture wearers experiencing slippage, soreness, or dietary limitations are strong candidates for a fixed solution that does not rely on adhesives or suction.

Good Overall Health

Active gum disease must be resolved before placement begins. A commitment to proper hygiene and follow-up care supports long-term implant success.

When bone loss is more significant, preparatory procedures such as bone grafting may be needed first. Patients with severe upper jaw bone loss may also be candidates for zygomatic implants as an alternative full-arch path. Imaging determines the right direction for every case.

What to Expect During All-on-4 Treatment

Treatment unfolds across several appointments, and we walk you through each phase clearly before anything is scheduled. The order and timing vary by case, but the path follows the same structure for most patients.

01
Consultation & CBCT Imaging
Dr. Patel reviews your full health history and takes 3D CBCT imaging to map your bone structure, confirm candidacy, and plan exact implant placement.
02
Preparatory Work
Any remaining teeth requiring extraction are removed. Bone grafting is completed if indicated. Sedation options are reviewed ahead of surgery day.
03
Implant Placement
Four implants are placed under anesthesia. A temporary full-arch prosthesis is attached the same day so you leave our Charlotte office with functional teeth.
04
Final Restoration
After a healing period of approximately three to six months, your temporary arch is replaced with the final fixed restoration. Complete aftercare instructions are provided before you leave.

Most patients return to light daily activities within a few days of surgery. Our team stays available throughout recovery and at every follow-up appointment to confirm healing is on track. Full-Arch Implant Solutions at Harmony in Charlotte

All-on-4 is the most common full-arch path, but it is not the only one. Depending on your anatomy, bone volume, and goals, Dr. Patel may discuss one of the following approaches during your consultation. Every plan begins with imaging, not assumptions.

  • Full-mouth dental implants offer a fixed and permanent alternative to removable dentures, rebuilding an entire upper or lower arch with results that integrate with the bone and do not rely on adhesives or suction for stability.
  • Teeth in a Day uses 3D imaging and a surgical guide to plan and place implants with a same-day provisional arch in a single appointment, reducing the number of surgical visits for qualifying patients.
  • Zygomatic implants anchor into the dense cheekbone rather than the jaw, making full-arch restoration possible for patients with significant upper jaw bone loss who are not candidates for conventional placement.

Dr. Patel performs all of these procedures in-house at our Charlotte location, which means your evaluation, surgery, and follow-up care all happen with the same surgical team from start to finish.
